You will need at least six… perhaps seven if the room is any larger than 16X12
The method that you use to attach the ply will depend upon the type of subfloor or joist system. If you’re attaching to a common 16 or 24" joist system below, you’ll nail on 8" centers on the perimeter and 12" in the field. If you want, you can apply liquid nails to help prevent ‘squeeks’ in the future. No need to screw if you use the proper nails, and are covering with a decent floor covering.
Good luck
You will need 6 sheets…recommend you use liquid nail as well as nails or preferably screws to install them…

The right answer is 6 sheets. 16′x12′= 192 sq ft
8′x4′ = 32 sq ft
6 x32= 192
Before screwing or nailing the boards down be sure there are no pipes or cables to pierce.
Good luck.
